Deputy CEO Update

Like all of you, our Members, we had a very busy but exciting year in ISQua. The most interesting for me, is the fact that all of our programmes are becoming more interdependent. We no longer discuss the Conference in isolation of Education or Accreditation. Equally Membership and the Journal contribute so much to all of the work we do in both developed and developing economies. Our small team here in Dublin, certainly understand the term “multitask”.

Combating Medication Errors
byDr. Salwa Ahsan, Pharm.D, Manager Pharmacy, Aga Khan University Hospital (Karachi – Pakistan)
Medication errors are common problem across the healthcare organizations irrespective of the state of economy of the country. In Pakistan where the infusion of technology is slow especially in the healthcare organizations, the problem becomes profound as the options to curb this nuisance become limited. Our Pharmacy Services also face this challenge and strive to prevent these errors.

Of more interest to pharmacy services are the “dispensing errors”. Wrong drug dispensing encompasses a great variety of sub-categories such as wrong drug, wrong strength, wrong dosage form, wrong quantity/volume etc. and each has a potential to lead to patient harm although severity can vary.

Considering IHF Membership for Accredited Hospitals
International Hospital Federation (IHF)
byEric de Roodenbeke, PhD C.E.O, IHF

In countries where accreditation is not mandatory either for health insurance plans or for the government, it is most often a decision driven by the will to implement a recognized quality improvement process and to receive a third party acknowledgment that provided care is up to international recognized standards. This choice is important in an environment of increasing globalization of healthcare supply especially for elective care.
